증례: Streptococcus pyogenes 감염에 의한 APT가 의심되는 경우(Streptococcal pharyngotonsilits)

S	F/17 내원 2일 전부터 fever, myaliga, sore throat 발생 C/C: sore throat(Onset: 2DA) C/S/R(-/-/-). 금일 아침 해열제 복용함 PHx DM/HTN/Tb/Hepatitis(-/-/-/-) medication: 금일 아침 - 해열제 복용
O	V/S: 112/69-113-38.6 Pharyngeal injection(+/+) tonsil enlargement: Grade II/II c exudate, Lt.
A	acute pharyngotonsilitis: R/O GABHS infection Modified Centor criteria (1) 체온 > 38℃ → 1점 (2)기침이 없음 → 1점 (3) 압통을 동반한 전방 경부 림프절 종대 → 0점 (4) 편도 비대 또는 삼출물 → 1점 (5) 연령 15-44세 → 0점 → Modified Centor criteria 3점으로 Streptococcus pyogenes 감염에 의한 APT 가능성이 높다.
P	항생제 투여 === Pamoxin cap. 500 mg (Amoxicillin) 3C #3 × 5days Soleton tab. 80 mg (Zaltoprofen) 3T #3 × 5days Tylenol 8hr ER Extended Relief 650 mg (Acetaminophen) 3T #3 × 5days Tantum Gargle 150 mg/100 mL (Benzydamine) 1BT #1 × 5days 2017년 국내 가이드라인(성인 급성 상기도 감염 항생제 사용지침)에서는 Modified Centor criteria 3점 이상인 경우 S. pyogenes RADT(rapid antigen detection test)가 가능한 경우에는 결과에 따라 항생제 투여여부를 결정하거나 S. pyogenes RADT가 불가능한 경우에는 항생제 투여를 권고하고 있다. 1차 항생제로 amoxicillin 10일 요법을 사용할 수 있다.
