- 신장 조직 검사: 사구체 메산지움 IgA 침착 확인

해설:

젊은 남성에서 상기도 감염과 동반되어 발생하는 육안적 혈뇨는 IgA 신증의 가장 흔한 임상 양상 중 하나다. 이를 "synpharyngitic hematuria"라고도 한다.
혈뇨는 대개 감염 후 1-2일 이내에 발생하며, 수일 내에 호전되는 경향을 보인다.
IgA 수치 상승은 진단에 도움이 될 수 있지만, 필수적인 소견은 아니다.
콩팥 생검은 IgA 침착을 확인하여 확진하는 데 필수적이다.

해설:

IgA 신증은 뚜렷한 증상 없이 건강 검진 등에서 우연히 발견되는 경우도 많다.
지속적인 현미경적 혈뇨와 함께 경미한 단백뇨가 나타날 수 있다.
IgA 수치는 정상일 수 있으며, 콩팥 생검을 통해 확진한다.
이러한 환자는 신기능 저하 위험을 평가하고, 단백뇨 정도에 따라 ACE 억제제 또는 ARB 등의 치료를 고려하며 경과 관찰한다.

해설:

IgA 신증은 일부 환자에서 신증후군(심한 단백뇨, 저알부민혈증, 부종, 고지혈증) 및 진행성 신기능 저하로 나타날 수 있다.
진단 시 이미 상당한 콩팥 손상이 진행된 경우가 있으며, 예후가 상대적으로 불량할 수 있다.
치료는 단백뇨 감소를 위한 ACE 억제제 또는 ARB 사용, 신기능 보존을 위한 면역억제제 치료(스테로이드, MMF 등)를 고려하며, 고혈압 및 고지혈증 등 동반 질환 관리도 중요하다.

IgA nephropathy, Haas subclass IV, Oxford classification system M1 E1 S1 T0 C0.

LM> The sections reveal renal cortex and medulla containing 38 glomeruli.

Three of them show total sclerosis. Remained glomeruli reveal marked

widening of mesangial matrix with mesangial cell proliferation.

Occasional neutrophilic infiltration is present.

Segmental sclerosis is noted in 4 glomeruli. There is no crescent.

Glomerular capillary walls are mostly thin and even.

Tubules show focal atrophy (less than 5% of cortical area).

Blood vessels are unremarkable.

IF> The sections reveal 8 glomeruli showing strong IgA and C3 deposits at

mesangium. Focal and segmental, mild fibrinogen deposits are present at

mesangium.

Comment: According go the Oxford Classification for IgA nephropathy, scores of M1 and S1,

respectively, indicate mesangial hypercellularity and segmental glomerulosclerosis,

and by themselves indicate a mildly increased risk of renal functional deterioration.

However, M1 and S1 together suggest a considerable risk of such deterioration.

Tubular atrophy (T) scores of T1 and T2 indicate progressively greater risk of renal

functional decline compared with T0, and augment the poorer prognosis associated

with M1 and S1. Endocapillary proliferation (E1), if present, suggests a lesion that

may improve with immunosuppressive therapy. (Reference: Kidney Int 76: 534~545

and 546~and 556, 2009).

해설:

신장 조직검사 결과 IgA 신병증(IgA Nephropathy, IgAN)으로 밝혀졌다.
IgAN은 위험도에 따라 치료방침을 결정하게 된다. 단백뇨가 UPCR 0.5
정도로 경미한 수준이고 콩팥기능도 정상이고 혈압도 ARB 투여로 잘 조절되고 있는 상태이므로 면역억제제 투여없이 보존적인 치료를 하면서 경과를 보기로 하였다.
다만 신장조직검사 결과 예후가 비교적 나쁜 형태임이 확인되었는데 추후에 신장소견이 악화될 가능성이 높아보인다.

해설:

nephrotic range의 proteinuria 뿐만 아니라 eGFR이 많이 감소되어 있다.
당뇨로 진단받은지 얼마되지 않았음을 감안하면 DM nephropathy보다는 다른 가능성이 보다 더 높아보인다.
이 때 당뇨망막병증 유무가 감별이 큰 도움이 된다. 안과진료를 권고하였다.

해설:

일반적으로 당뇨병성 망막병증이 당뇨병성 신증보다 먼저 발현되는 경향이 있다.
- 혈관의 크기 및 구조: 망막의 미세혈관이 신장의 사구체 모세혈관보다 직경이 더 작고 구조적으로 더 취약하여 고혈당의 영향을 더 일찍 받을 수 있다.
- 질병 진행 양상: 망막병증은 비교적 초기에 미세동맥류나 출혈 등 육안으로 확인 가능한 병변을 형성하는 반면, 신증은 초기에 자각 증상이나 명확한 신기능 저하 없이 미세알부민뇨와 같은 생화학적 변화로 시작된다.
본 증례에서는 안과진료 결과 망막병증은 없는 것으로 확인되었다. DM nephropathy보다는 chronic glomerulonephritis(CGN) 등의 다른 가능성을 탐색해야 한다.

Ten of them (425) show total sclerosis. Remained glomeruli reveal marked widening of

mesangial matrix with segmental mesangial cell proliferation.

Segmental sclerosis is present in 7 glomeruli. Glomerular capillary walls of nonsclerotic

segments are thin and even. Tubules reveal atrophy (about 30% of cortical area)

associated with interstitial lymphocytic infiltrate. Arterioles show frequent hyaline deposits.

IF> The sections reveal 9 glomeruli showing strong IgA, mild fibrinogen deposits at mesangium.

Mild linear IgG deposits are present along the tubular basement membranes.

Comment: According go the Oxford Classification for IgA nephropathy, scores of M1 and S1,

respectively, indicate mesangial hypercellularity and segmental glomerulosclerosis,
