후천질환(Acquired)

혈소판증가증의 대부분은 후천적이고 이차성 혹은 일차성으로 발생
1. 일차성 질환: clonal bone marrow (myeloproliferative) disorder (→ 여기에 essential thrombocythemia가 포함)
2. 이차성 질환: 반응성 질환(2차성 혈소판증가증)

반응성(이차성)혈소판증가증[Reactive (secondary) thrombocytosis]

일반인구에서 혈소판증가증의 가장 흔한 원인은 반응성 혹은 이차성 혈소판증가증임
내과 혹은 외과 환자 732명을 대상으로 한 연구(혈소판 수 ≥500,000/mm3)
- secondary thrombocytosis → 643 (88%)
- 가장 흔한 기저원인: tissue damage due to major surgery, infection, cancer, and chronic inflammation
280명의 입원환자를 대상으로 한 연구(혈소판수 ≥1,000,000/mm3): 231 (82%): reactive thrombocytosis; 11 (4%): thrombocytosis of uncertain cause; 38 (14%): myeloproliferative disorder

이차성(반응성) 혈소판증가증의 원인[!^2]

진단의 원칙

클론성(clonal) 혈소판증가증과 이차성(반응성) 혈소판증가증을 감별해내는 것이 키포인트
클론성(clonal) 혈소판증가증과 이차성(반응성) 혈소판증가증을 명확하게 감별할 수 있는 진단적 소견(diagnostic finding)은 현재로선 없음
- 임상소견 및 검사소견만으로 이 둘을 감별하기는 어려움
- 혈소판 수치 상승정도로는 이 둘을 명확하게 감별할 수 없음
- 이 둘 사이에는 원인, 병태생리, 임상양상이 기본적으로 다름

진단기준

혈소판증가증을 보일 수 있는 골수증식종양(myeloproliferative neoplasm)에 속하는 본태혈소판증가증(Essential thrombocytosis, ET), 진성적혈구 증가증(Polycythemia vera, PV), 일차골수섬유증(Primary myelofibrosis, PM)에는 각각 진단기준이 있음

지속적인 혈소판증가증을 보이는 경우 진단흐름도[!^1][!^3]

크게 가성 혈소판증가증과 반응성 혈소판증가증을 배제하는 과정과 clonal thrombocytosis에 대한 감별진단 과정(본태성 혈소판증가증을 다른 골수증식질환 혹은 골수이형성질환과 감별)으로 나눌 수 있는데 전자는 일차진료에서 수행가능한 과정이지만 후자의 과정은 혈액 전문의(hematology)에 의뢰해야 할 듯 하다.
