스테로이드 주사

가장 흔하게 사용되는 치료법
흉터 내에 triamcinolone을 직접 주사하여 섬유아세포의 성장을 억제하고 염증을 줄인다.
흉터의 크기를 줄이고 가려움증을 완화하는 데 효과적이다.
여러 차례 반복 치료가 필요할 수 있다.

작용 기전

콜라겐과 글리코사미노글리칸(glycosaminoglycan)의 합성을 줄이고 섬유아세포 증식을 억제한다.
또한, iNOS(유도성 산화질소 합성효소) 전사를 억제하여 콜라겐 생성을 감소시키고, 콜라게나제 억제제인 알파-2-마크로글로불린(alpha-2-macroglobulin)의 합성을 억제함으로써 흉터 치료 효과를 나타낸다.

시술 방법

용량: 일반적으로 triamcinolone acetonide(TAC)를 10–40mg 용량으로 사용하며, 최대 5mg/cm²를 넘지 않아야 한다.
주사: 0.9% NaCl 또는 리도카인과 1:2에서 1:4 비율로 희석하여 사용하거나 희석 없이 투여할 수 있다. 주사는 반드시 흉터 내부에 정확히 주입해야 한다. 주사 부위가 하얗게 변색되는 것은 주입이 잘 이루어졌음을 의미한다.
간격: 필요에 따라 3-4주 간격으로 주사를 반복할 수 있다.
부작용 완화: 낮은 농도(10 mg/ml)로 시작하여 점진적으로 농도를 높이면(20mg/ml, 40mg/ml) 부작용과 재발 위험을 줄일 수 있다. 주사 전 냉동수술(cryosurgery)을 가볍게 시행하면 부종이 생겨 약물 주입이 더 쉬워지고 통증도 완화된다.

참고: 예방적 치료

수술 중 흉터 재발을 방지하기 위해 상처 가장자리에 1cm당 1mg의 triamcinolone을 직접 주사하면 효과가 좋고 국소 부작용도 적다.

냉동수술(Cryosurgery)

작용 기전

냉동수술의 효과는 주로 저온으로 인한 미세순환 변화, 혈전증, 그리고 그에 따른 허혈성 세포 사멸에 기반한다.

부작용

약 4주에 이르는 긴 회복 시간과, 추위에 민감한 멜라닌 세포의 파괴로 인해 빈번하게 발생하는 (가역적인) 색소 탈실이 있다.

시술 방법

냉동수술에는 크게 두 가지 기본적인 접근법이 있다.

짧은 냉동수술: 특히 트리암시놀론 아세토나이드(TAC) 주입을 용이하게 하기 위해 TAC와 병용할 때 사용된다.
강한 냉동수술: 조직 전체를 얼리는 방식으로, 스프레이 또는 접촉식 냉동법이나 병변 내 냉동수술로 시행할 수 있다. 대부분의 경우, 흉터가 충분히 평평해질 때까지 4-6주 간격으로 시술을 반복해야 한다.

병변 내 냉동수술

여러 가지 한계 때문에 병변 내 냉동수술은 자주 사용되지 않는다.

켈로이드 크기에 따라 여러 개의 바늘이 필요할 수 있으며, 큰 켈로이드를 치료하는 데 시간이 오래 걸린다.

국소 마취 후 무균 상태에서 이중 속이 빈 바늘을 통해 켈로이드 내부에 질소를 주입한다. 그런 다음 병변 주변 3-5mm의 후광(halo)과 함께 켈로이드 전체를 얼린다. 켈로이드의 완화는 약 4~6개월 이내에 나타난다.

환자에게 물집이 생겨 진물이 나는 상처로 진행될 수 있음을 미리 알려주어야 하며, 소독 치료가 필요할 수 있다.
다음 치료는 이전 치료로 인해 생긴 결함이 완전히 치유된 후에만 시행해야 한다.
또한, 피부색이 어두운 환자의 경우 색소침착(hyperpigmentation) 또는 색소탈실(hypopigmentation)이 발생할 수 있음을 경고해야 한다.
활동성 켈로이드는 스프레이 냉동수술 후 악화될 수 있으므로, 이 경우에는 이후에 트리암시놀론 주사와 같은 병용 요법이 도움이 된다.

압박 요법 (Pressure Therapy)

작용 기전

국소적인 압력은 모세혈관의 혈류를 감소시키고 콜라겐 성숙을 촉진하여 흉터를 평평하게 만든다.

부작용

열감, 땀, 팔다리 부종, 피부염, 압박으로 인한 표피 미란 및 궤양 등이 발생할 수 있다.

시술 방법

압박 요법은 탄력 있는 재료를 사용하여 가능한 한 빨리 (즉, 상피 재형성이 완료된 후) 시작해야 하며, 병적인 흉터가 발생하기 쉬운 환자의 경우 예방적으로도 시행할 수 있다.
필요한 압력은 20~30mmHg(압박 등급 II에 해당)이며, 하루 24시간 내내 유지해야 한다.
압박 요법은 보통 압박복이나 붕대를 사용하며, 특정 부위에는 투명한 플라스틱 마스크나 압박 단추를 사용하기도 한다.
선호되는 장신장성(long-stretch) 재질로 만든 압박 붕대는 짧은신장성(short-stretch) 재질보다 둘레의 미세한 변화(예: 부종)에 덜 영향을 받는다.
따라서 허용 가능한 최대 압력에 도달하는 시간이 훨씬 더 길고, 반대로 여전히 효과적인 최소 압력을 놓칠 가능성이 적다.
맞춤 제작된 압박 붕대의 경우, 재질의 특성상 약 6개월 후에 압력이 감소한다.
압력은 6~24개월의 치료 기간 동안 서서히 낮출 수 있다.
수술 후 예방 목적이라면 최소 6~24개월 동안 치료를 지속해야 한다.

환자의 노력이 많이 필요하며, 환자와 의사 모두에게 꾸준한 실천이 요구된다. 치료 비용이 비쌀 수 있으므로, 초기 상태와 시간이 지남에 따른 변화를 주의 깊게 기록해야 한다.

레이저: 부분 박피 레이저 치료 (Fractional Ablative Laser Treatment)

레이저 장비

박피 레이저(Ablative lasers): CO₂, Er:YAG, Er:YSGG and thulium lasers

특정 부위에 국한하여 사용하거나, 분할된 방식으로 사용할 수 있다. 후자의 방법은 박피 구역 사이에 일부 생체 조직을 남겨두어 부작용 발생률을 낮추면서도 효과를 확실하게 보장한다.

작용 기전

박피 채널(ablation channels) 주변 표피에서 생성된 열충격 단백질을 포함하는 특정 시공간적 상처 치유 과정이 일어나면서 표피와 진피의 재구조화가 유도된다.
이로 인해 세포 및 결합 조직 구조, 혈관, 신경, 그리고 국소 면역 체계가 정상화된다.
이 레이저를 흉터 치료에 사용할 경우, 흉터 두께뿐만 아니라 가려움증, 붉은 기, 밀도, 색소 침착 등이 개선되는 경우가 많다. 이는 예를 들어, 구축으로 인한 기능적 측면이 향상되는 결과를 가져올 수 있다.

부작용

박피 레이저 치료는 항상 피부색이 어두워지는 현상(bronzing)을 유발하며, 높은 에너지로 시술 시 표피 미란, 진물, 딱지, 지속적인 붉은 기가 생길 수 있다.
Er:YAG 레이저는 열을 거의 발생시키지 않는 반면, CO2 레이저 치료 후에는 색소 침착 및 탈색이 더 강하고 오래 지속되는 경향이 있다.

시술 과정

부분 박피 레이저(Ablative fractional laser, AFXL)는 봉합사 제거 직후 또는 가능한 한 빨리 시작한다.
4주 간격으로 최소 6회 이상 시술하는 것이 권장되며, 이후 치료를 종료할지 또는 추가 시술을 진행할지 결정한다.
흉터 조직에 생성된 박피 채널은 약물 투여(레이저 보조 약물 전달, LADD)를 위해 활용될 수도 있다.

부분 박피 레이저 치료는 기술적으로 간단하고 빠르게 수행할 수 있다.
단독으로 사용하거나, LADD(레이저 보조 약물 전달)를 통해 TAC/5-fluorouracil과 병행하면 흉터의 가려움증, 붉은 기, 밀도, 색소 침착, 두께 중 한 가지 이상을 개선하며 환자의 주관적인 미용적 만족도도 향상시키는 결과를 낳는다.

비박피 레이저 치료(Non-ablative laser treatment)

Pulsed Dye Laser(PDL)

작용 기전

레이저 응고는 혈관 공급을 감소시켜 저관류와 저산소증을 유발하고, 그 결과 병적 흉터가 퇴행한다.
또한, flash lamp-pumped pulsed dye laser (FPDL) 사용 후 변형 성장 인자(TGF)β1 발현 감소, 섬유아세포 증식 감소, MMP-13(콜라게나아제 3) 상향 조절, 섬유아세포 세포자멸사 유도, 그리고 ERK와 p38-MAP 키나제 활성 상향 조절이 보고되었다.

부작용

자반(멍)이 자주 발생하며 최대 14일 동안 지속될 수 있다.
레이저 에너지 밀도와 환자의 피부 색소 침착 정도에 따라 물집과 딱지가 생길 수도 있다.
어두운 피부를 가진 사람에게는 지속적인 과색소 침착이 더 흔하지만, 585nm 파장보다 595nm 파장에서 더 드물게 관찰된다.

시술 과정

환자의 피부 색소 침착 정도에 따라, 585nm FPDL을 사용하여 에너지 밀도 약 5.5~7.5J/cm²에 5~7mm 스팟 직경으로 시술하거나, 10mm 스팟 크기에 약 4.5~5.5J/cm²로 시술하는 것이 일반적이다.
약 6~8주 간격으로 최소 2회 이상의 치료 세션을 권장한다.

Nd:YAG 레이저 (1064nm)

작용 기전

진피 깊은 곳의 혈관 손상이 섬유아세포의 콜라겐 생성을 감소시킨다.

부작용

PDL과 동일한 부작용이 있을 수 있으며, 통증이 매우 심하다.

시술 과정

치료 프로토콜이 아직 충분히 표준화되지 않았습니다. 시술 시 '중복 조사'를 피해야 한다.

IPL (Intensed Pulsed Light)

작용 기전

작용 기전은 혈관 필터를 사용하는 PDL과 동일한 것으로 추정된다.

부작용

PDL과 동일한 부작용이 있으며, 과도한 열 손상이 있을 경우 상태가 악화될 위험이 있다(특히 피부 타입 V 및 VI의 경우).

시술 과정

치료 프로토콜이 아직 충분히 표준화되지 않았으며, 사용되는 레이저에 따라 크게 달라진다.

실리콘 패드/겔/시트

흉터가 생긴 직후부터 최소 3개월 이상 꾸준히 사용해야 한다.

수술적 치료

외과적 절제
- 켈로이드나 비대흉터가 큰 경우, 흉터 조직을 잘라내는 수술을 시행할 수 있다.
- 그러나 이 방법은 흉터가 재발할 위험이 매우 높기 때문에, 수술 후에는 스테로이드 주사나 방사선 치료를 병행하는 것이 필수적이다.

기타 치료

방사선 치료: 수술로 켈로이드를 제거한 후, 방사선 치료를 통해 재발을 억제
