치료원칙

위암의 표준치료는 개복을 통한 위 절제술과 광범위 림프절절제술이다[!^1].
절제가 가능한지 여부가 중요하다.
- 절제가능한 경우 내시경적인 절제가 가능하면 당연히 상대적으로 덜 침습적인 내시경적 절제술(ESD)을 시행하게 된다. ESD가 불가능하면서 수술을 시행한다.
- 절제가 불가능한 경우 고식적(palliative) 치료를 시행한다.

내시경 치료

내시경 치료 방침: 내시경 치료의 적응증

조기위암의 일부, 특히 점막에 국한된 암 (M cancer) 중 크기가 작은 분화암은 림프절 전이의 가능성이 매우 낮으므로 내시경을 이용한 국소치료만으로 완치가 가능하다.
크게 고식적 적응증/절대적응증(conventional/absolute indication)과 확대 적응증(expanded indication)으로 구분 가능

수술한 환자의 lymph node status를 분석하여 림프절 전이가 거의 없는 병소의 특성을 반영한 것이 내시경 치료(주로 ESD)의 적응증이라고 볼 수 있습니다. 참고로 ESD와 EMR의 적응증은 같다고 보시면 됩니다.

고식적 적응증/절대적응증(conventional/absolute indication)

내시경 절제술 방법

내시경으로 점막병소를 자르는 치료인 내시경 절제술(endoscopic resection, ER)에는 여러 가지 방법이 있는데 대표적으로 2가지 방법, 즉 내시경 점막하 절제술(endoscopic submucosal dissection, ESD), 내시경 점막 절제술(endoscopic mucosal resection)이 주로 사용됩니다. 병소의 하단을 절제한다는 기본 원리는 동일합니다.("endoscopic resection (ER) = EMR + ESD")

내시경 점막 절제술(endoscopic mucosal resection)

EMR-P (endoscopic mucosal resection with precutting): 보통 EMR에 P를 붙여 EMR-P (endoscopic mucosal resection with precutting)라고 부릅니다.

needle knife나 IT knife 등을 이용하여 병소 주변을 360도 cutting 한 후, snare를 이용하여 병소 하단의 점막하조직을 절제하는 방법

내시경 점막하 절제술(endoscopic submucosal dissection, ESD)

병변의 절제범위를 더 넓고, 더 깊게 시술이 가능하여 조기 위암의 큰 병소에 대한 일괄절제(en bloc resection)가 가능하기 때문에 현재 EMR을 대신하여 널리 사용되고 있는 방법
ESD는 EMR-P와 마찬가지로 병소 주변을 360도 cutting한 후, snare를 이용하지 않고 needle knife나 IT knife를 이용하여 점막하 조직을 직접 dissection 하는 방법
단점: 시간이 오래 걸리고 합병증 발생률이 높다.

보통 EMR과 ESD는 서로 다른 시술법으로 간주되곤 하는데 이는 ESD가 conventional EMR에 비하여 난이도가 높고, 시술시간이 길고, 보조 인력이 많이 필요하며, 기구와 장비가 상당부분 다르기 때문입니다.

수술 방법
- 위상부 위암: 위전절제술뿐 아니라 근위부 위절제술도 생존율, 영양, 삶의 질 측면에서 상부 조기 위암에 대해 시행될 수 있는 수술방법이다.
- 위중부 위암: 유문 보존 위절제술 및 원위부 위절제술은 수술 후 장기 생존율, 영양 및 삶의 질을 고려하여 위중부 조기 위암 환자에게 시행할 수 있다.
- 위중하부 위암: 원위부 위절제술 후 위십이지장문합술 및 위공장문합술(Roux-en-Y 및 루프)가 권고된다. 여러 유형의 재건술간 장기 생존율, 기능 및 영양 측면에서 차이가 없다.
림프절 절제술
- D1+ 림프절 절제술은 장기 생존 측면에서 조기 위암(cT1N0) 수술에 권고된다.
- 비장문 림프절 절제를 위한 예방적 비장절제술은 위상부 진행 위암의 근치적 절제 시 권고되지 않는다.
- 식도위접합부 선암에서 종격동 하부 림프절 절제는 수술 후 합병증을 증가시키지 않으면서 종양학적 결과를 개선하기 위해 수행할 수 있다.

수술: 위 절제 (Gastric resection)

위 절제 범위: 안전한 절제연의 확보가 필수적!
- 조기 위암의 경우: 2cm 이상
- 진행 위암의 경우: 3-5cm 이상
절제연이 확보되지 않은 경우나 그 외 필요시 수술 중 동결절편 검사를 시행하여 암 침윤 여부 확인 가능

위아전절제술(distal subtotal gastrectomy, 2/3 절제)

중상부 위암

위전절제술(total gastrectomy), 주위의 광범위한 림프절 절제술을 함께 시행

축소수술 또는 기능보존 수술[!^1]

유문보존 위절제술(pylorus-preserving gastrectomy), 국소절제술 (local resection), 구역절제술(segmental resection), 근위부 위절제술(proximal gastrectomy)①

① 근위부 위절제술은 상부에 위치하는 조기 위암에서 주로 시행되며 역류성 식도염이 발생할 위험성이 있어 주의가 필요하다.

권고

근치적 절제가 가능한 위암의 표준 치료는 수술이다.
근치적 수술이란 원발 병소의 완전 절제와 위 주위의 광범위한 림프절 절제술을 시행한 후 위장관 재건을 시행하는 것이다.
근위부 위절제술은 제한된 적응증에서 위전절제술을 대체할 수 있다.

권고: 원위부 위절제술 시행 후 위십이지장, 위공장, 루와이 문합법은 모두 사용할 수 있다.

수술: 합병절제

원발병소가 인접장기로 직접 침윤하였거나 원격 전이 병변이 있는 경우 완전 절제 또는 증상 완화를 위해 침범된 장기의 합병 절제 시행 가능

원발암이나 전이성 국소림프절이 주변 인접장기를 침윤하였을 때 완전절제를 위해 합병절제 시행 가능
위 주변 인접장기: 췌장, 비장, 간, 횡행결장, 부신, 신장, 담낭, 횡격막, 소장, 복막, 후복막
비장문 림프절 절제는 상부 위 대만측에 위치한 위암이 고유 근층 이상을 침윤한 경우나 비장문 부근(splenic hilum)의 림프절 전이가 의심될 경우 비장합병절제 시행 가능

원격 전이장기의 절제

난소, 간 , 복막, 등의 원격전이가 있는 위암환자에서 전이병소의 합병절제 선택적으로 시행 가능
