개요

과다호산구증후군(Hypereosinophilic syndromes, HES)이란?[!^1][!^2]

다음과 같은 특징을 가지는 heterogeneous group

1. persistent eosinophilia >1500 eosinophils/μL + end organ damage or dysfunction

2. in the absence of secondary causes of eosinophilia

6개월 이상 지속되는 말초혈액의 호산구의 증가(1,500개/mm3 이상)
기생충 감염이나 알레르기 질환 등의 호산구의 증가를 유발할 수 있는 다른 질환이 없을 것
장기 침범의 증거가 있을 것

설사 말초혈액 내 호산구 수의 증가가 있더라도 아무 증상이 없다면 제외된다.

organ damage and/or dysfunction attributable to tissue HE and
exclusion of other disorders or conditions as major reason for organ damage

peripheral blood HE

말초혈액검사에서 1 μL당 1,500개 이상의 호산구증가증이 4주 간격으로 2회 이상 확인되는 경우
(즉각적인 치료를 요하는 장기 손상이 발생한 경우에는 기간과 관계없이 진단 가능)

tissue HE

골수 조직에서 호산구분율이 전체 유핵세포의 20% 이상을 차지하는 경우
병리학적으로조직 내 광범위한 호산구 침윤이 관찰되는 경우
호산구의 침윤 유무에 상관없이 조직에서 호산구 과립 단백질의 뚜렷한 침착이 관찰되는 경우
