개요

임상양상

호산구식도염의 진행에 따른 임상양상

Inflammatory phase를 지나 fibrotic phase로 진행

소아는 inflammatory phase에 발견되는 경우가 많고, 성인은 fibrotic phase에서 발견되는 경우가 많다[!^1].

진단기준

조직검사에서 eosinophilia가 high power field 당 15개 이상인 것이 가장 중요한 기준이다. 정상 기준은 5개 이하이다. 드물게 10개까지 정상으로 간주하는 전문가도 있다[!^1].

식도에 국한된 경우만을 EoE로 부르고 있다. Eosinophilic gastroenteritis가 있으면서 식도의 호산구 침윤이 있는 경우는 EoE로 부르지 말도록 정의하고 있다[!^1].

EoE Diagnostic Criteria

Symptoms of esophageal dysfunction
- Concomitant atopic conditions should increase suspicion for EoE.
- Endoscopic findings of rings, furrows, exudates, edema, stricture, narrowing, and crepe paper mucosa should increase suspicion for EoE.

≥15 eos/hpf (∼60 eos/mm²) on esophageal biopsy
- Eosinophilic infiltration should be isolated to the esophagus.
Assessment of non-EoE disorders that cause or potentially contribute to esophageal eosinophilia

내시경 소견

호산구 식도염 내시경 소견[!^1]

1) Stacked circular rings ("feline" esophagus)
2) Stricture (particularly proximal stricture)
3) Attenuation of the submucosal vascular pattern
4) Liner furrowing that may extend length of the esophagus
5) White papules (representing eosinophilic microabscess)
6) Small caliber esophagus

EoE의 특징적인 내시경소견 및 중증도를 평가하기 위한 scoring system[!^3]

<증례>

a loss of vascularity (E2)

significant rings obstructing passage of a regular adult endoscope (R3)

mild exudates seen vaguely on middle and right picture (further exudates not pictured here) (E1)

mild furrows on left within no depth (F1)

a significant stricture (S1)

Final total score for the Endoscopic Reference Score is 8.

치료 후에는 longitudinal furrow는 healing이 되는데 circular ring은 그대로인 경우가 많다고 한다. Fixed circular ring은 이미 fibrotic stage이기 때문인 것 같다[!^1].
