급성 유양돌기염은 급성 중이염의 합병증으로 주로 1~3세 사이에 호발
급성 중이염으로 장기간의 항생제를 복용하던 중에 지속적인 이통, 발열이 동반되면서 고막 발적, 화농성 이루, 이개 후부 발적 및 이개 돌출이 관찰되면 급성 유양돌기염으로 진단 가능

항생제 내성균이 증가하고 환자의 치료 순응도가 낮거나 불충분한 용량 및 불충분한 기간의 항생제 처방 또는 부적절한 항생제의 처방으로 급성 유양돌기염 및 골막하 농양, 미로염, 안면신경마비, 뇌수막염, 뇌농양, 그리고 S상 정맥동 정맥혈전염 등과 같은 합병증은 일정 수준 이하로 더 감소하지 않고 있다.

발생기전
- 급성 중이염으로 중이강 내 점막에 부종 및 증식이 발생하여 유양돌기구가 막히면서 주변 골의 탈무기질화가 일어나고, 유양돌기의 배액이 불가능해지면서 염증이 유양돌기의 도출정맥(emissary vein)을 통해 점막골막(muco-periosteum)을 따라 퍼진 후 골염으로 발전하여 유양봉소 격벽을 녹이고 단백성 기질의 파괴가 일어나 인접 유양봉소간의 융합이 일어나게 된다.
- 이후 농양이 형성되기도 하며 농양의 파급 경로에 따라 골막하 농양, Bezold 농양, 추체염, Citelli 농양으로 나뉘기도 한다. 그 외에 미로염 및 안면 신경 마비, 두개 내 합병증이 발생할 수 있다.

원인 병원체

1. acute otitis media의 합병증으로 발생한 경우

Streptococcus pneumoniae (most common)

Streptococcus pyogenes

Haemophilus influenzae

Moraxella catarrhalis

2. 만성 중이염의 합병증으로 발생한 경우

상기 병원체 +

Staphylococcus aureus

Pseudomonas aeruginosa

acute mastoiditis를 유발하는 3가지 주요 병원체: S. pneumoniae, S. pyogenes, Staphylococcus aureus

항생제 요법[!^4][!^5][!^3]

3~6주간의 항생제 정주 치료와 고막절개, 절개 배농술로 대부분 치료 가능

1차 치료제

3rd generation cephalosporin
- Ceftriaxone 2 gm IV q24h OR
- Cefotaxime 1-2g IV q4-8h
beta-lactam/betalactamase inhibitor agents
- Piperacillin-Tazobactam 3.375 gm q6h or 4.5 gm IV q8h

대체 치료제

severe beta-lactam allergy
- Levofloxacin 750 mg IV q24h
- Moxifloxacin 400mg IV q24h

S. aureus가 배양되는 경우 → 감수성에 따라 Vancomycin or Nafcillin 투여
