2주 전 무렵부터 보호자(따님) 듣기에 말투 어눌함 있었으나 환자는 호소하는 특이 증상 없어 그냥 지켜보았다고 함. 3일 전 간헐적으로 단어가 선뜻 떠오르지 않을 때가 있고 환자 느끼기에도 말 어눌해지고, 보호자 듣기에도 말 유창함이 평소보다 떨어졌다고 함. 그렇지만 큰 불편함은 없어 경과관찰하다가 내원일 점심식사 후 낮 1시부터는 말 급격히 어눌해져 남편이 말을 못 알아듣게 되어 응급실 내원. 상, 하지 위약감은 없었음

- 신경학적 장애가 자발적으로 신속히 호전되지 않는가? → 신경학적 장애가 거의 정상화된 상태이다.

② minor stroke에서는 급성기에 aspirin 단독투여보다는 aspirin과 clopidogrel를 병용투여할 경우 secondary stroke prevention에 유리한 것으로 알려져 있다.

③ 심장탓(예. 심방세동)인지를 파악하는 게 핵심이다. 핵심검사는 심장초음파, holter monitoring 등이다.

급성기 허혈성 뇌졸중에서는 혈압을 너무 낮추지 말아야 한다.

정맥 내 혈전용해치료나 혈전제거술을 시행하지 않은 급성기 허혈성 뇌졸중 환자는 혈압이 220/120 mmHg 이하인 경우 고혈압치료가 필요한 다른 합병증이 없다면, 고혈압 치료를 시행할 필요가 없다.(특히 처음 48-72 시간 이내에는 고혈압치료로 사망이나 중증장애가 감소하는지 확실하지 않아 이를 권고하지 않는다.)

mRS 4	Moderately severe disability; unable to walk without assistance, and unable to attend to own bodily needs without assistance; 증상이 심해서 돌보아줄 사람이 필요하긴 하지만 밤낮으로 돌보아줄 사람이 필요한 건 아니다. 부축을 받아야 걸을 수 있다. 기본적인 일상생활(basic activities), 옷 입기, 식사하기 및 화장실 가기에도 도움이 필요하다.
mRS 5	Severe disability; bed-ridden, incontinence and requiring constant nursing care and attention; 증상이 심해서 밤낮으로 돌보아줄 사람이 필요하다. 부축을 해도 걷지 못할 정도로 계속 누워서 지내거나 앉아서 지내야 되는 상황 대소변도 계속 다른 사람이 받아내야 한다.
mRS 6	Dead; 사망

① mRS는 환자의 전반적인 기능적 예후(global functional outcome)을 평가하는 데 유용한 척도로 평가점수는 0-5 단계로 구성되며 6은 사망을 의미한다. mRS는 일상생활의 독립적 수행능력과 타인 도움의 필요 정도를 기준으로 전반적 기능을 평가하는데 mRS가 0-2점인 경우 예후가 양호하며 3점 이상인 경우 예후가 불량함을 시사한다.

고혈압으로 항고혈압제를 드시던 분으로 뇌졸중 급성기 때 일단 항고혈압을 중단했었음. 급성기 지난 시기에서 BP가 높게 측정되고 있어 2차 예방 측면에서 혈압을 조절해줄 필요가 있다.

② 급성기 동안 항혈전제 투여는 항혈소판제 combination (aspirin + clopidogrel)로 가다가 급성기가 지나면 clopidogrel monotherapy로 전환해서 평생 투여하게 된다.

- Pharyngeal phase에서 모든 검사 성상에서 vallecular sinus 및 pyriformis sinus의 moderate retention 보임. pharyngeal reflex 저하된 소견 관찰되며 initiation delay 보임. pharyngeal muscle 의 coordination 잘 되지 않음. liquid 성상에서 penetration 및 aspiration 소견 관찰됨
esophageal phase
- upper esophageal sphincter opening 및 esophageal relaxation intact 함
AP view: 양쪽 vallecular sinus 의 동일한 residue 관찰됨

- compensation maneuver (double & multiple swallowing)
- swallowing exercise (Mendelson maneuver, Effortful swallowing)
4주 후 VFSS F/U

Pharyngeal phase에서 vallecular sinus 및 pyriformis sinus의 moderate retention 보이며, 이는 double swallowing maneuver에 의해 partial clearing 되는 모습 보입니다. 검사 지속 시, endurance 떨어지는 모습 보이며, dysphagia tx. 및 vital stim 치료 필요합니다.

Liquid 섭취 시 penetration 및 aspiration 보이나 cough reflex 관찰되지 않고 있습니다. silent aspiration에 대하여 위험성 warning 해 주시고, liquid는 점도증진제를 이용하여 섭취가 필요합니다.

disease onset 고려하여, dysphagia tx. 지속하며 short term VFSS f/u 필요합니다.(외래 f/u 해 주십시오)
