강도에 따른 분류

Vasoconstrictor assay (혈관수축능 검사), Mitotic index suprresion method (분열 억제지수 측정법), Atrophogenic potential method (위축 잠재력 측정법) 을 이용하여 외용 스테로이드의 상대적 역가를 측정하여 분류할 수 있다
유사한 역가를 가진 약물로 나누는 방법은 크게 3가지가 있다.
- 3단계 – high/intermediate(medium)/low 로 나누는 방법
- 4단계 – very potent/potent/moderate potent/mild 로 나누는 방법
- 7단계 – 가장 강한 역가의 Class I ~ 가장 약한 역가의 Class VII 로 나누는 방법

제형에 따른 분류

연고(Ointment)
- 장점: ① 다른 제형보다보다 더 높은 윤활성/밀폐성 제공. ② 밀폐성향은 스테로이드흡수에 도움을 준다.
- 단점: ① 미끈거리는 느낌. ② 환자 만족도와 순응도가 떨어지는 편
크림(Cream)
- 일반적으로 동일한 약제의 연고 제형보다 역가가 낮다.
- 외용 스테로이드제의 대부분이 크림의 형태로 시판된다
- 장점: ① 물과 기름을 혼합하여 유화시킨 것으로 물에 잘 씻길 수 있어서 편리하다. ② 진물이 흐르는 경우에도 사용 가능. ③ 윤활능력(lubricating qualities) 우수. ④ 피부침투능이 좋아 미용적인 장점을 보임
- 단점: ① 밀폐능은 우수하지 못하다. ② 유효 약물 성분 외에도 다수의 부형제를 함유하고 있어서 자극이나 알레르기가 발생할 가능성이 있다.
로션(Lotion)
- 모든 국소 스테로이드 제형 중에서 젤과 더불어 기름성분이 가장 적고 밀폐효과도 가장 떨어진다.
- 알코올을 포함
- 진물이 나는 부위에 건조효과 제공
젤(gels)
- 반고형의 묵과 같은 형태의 용액으로 피부에 발랐을 때 액화된다.
- 모든 국소 스테로이드 제형 중에서 로션과 더불어 기름성분이 가장 적고 밀폐효과도 가장 떨어진다.
용액 (Solution)
- 장점: ① 수분이 있으므로 건조 작용을 가지고 있음. ② 진물이 나는 피부 질환에 효과적
- 단점: ① 용액 내에 함유된 성분에는 자극성을 가진 성분이 있다. ② 짓무르거나 갈라진 피부 병변에 사용하면 따가운 증상이 발생할 수 있다.
