1950년경 개발된 corticosteroid의 사용으로 루푸스의 치료에 서막이 열렸으며, cyclophosphamide의 사용은 루푸스 치료에 있어 획기적인 변화를 가지고 왔다. 이후 효과적인 질병 활성도 조절 및 신장 투석에 기인하여 루푸스 환자의 10년 생존율은 약 90-95%까지 보고되었다.

비스테로이드성 소염제/항말라리아제

일반적으로 경한 루푸스 환자에서 항말라리아제와 비스테로이드성 소염제가 자주 동시에 사용된다.
특히 열, 관절염, 근육통, 장막염(serositis)의 치료에 비스테로이드성 소염제가 일차적으로 사용된다.
- Serositis의 경우 일차적으로 NSAIDs를 사용한다. 이에 반응이 없을 경우 corticosteroid를 사용해 볼 수 있다.

항말라리아제와 비스테로이드성 소염제를 사용해서 관절염이나 피부 발진을 치료하지 못할 때
비교적 저용량의 스테로이드(low dose corticosteroid)가 사용된다.
주요 장기를 침범하여 생명을 위협할 수 있을 경우에 고용량의 스테로이드를 사용한다.
(예: 중증 자가면역성 용혈성 빈혈, 루푸스 신염, 폐간질염(pneumonitis), 심한 심외막염, 중추 신경계를 침범한 신경정신루푸스)
