요실금이란?

우선 일시적 요실금과 만성 요실금으로 분류
만성 요실금은 임상 증상에 따라 흔히 다음과 같이 분류: 복압성(긴장성, stress), 절박성(urge), 혼합형(mixed), 범람성(overflow), 기능성 요실금(functional incontinence)
복압성, 절박성, 혼합형 요실금이 대다수를 차지
성별과 연령대에 따른 요실금 발생 형태
- 중년 여성: 복압 요실금(m/c)
- 노인: 혼합 요실금(m/c)
- 남자: 절박 요실금 > 혼합 요실금 > 복압 요실금

Delirium

Infection (acute urinary tract infection)

Atrophic vaginitis

Pharmaceuticals

Psychological disorder, especially depression

Excessive urine output (예. hyperglycemia)

Reduced mobility (i.e., functional incontinence) or reversible (예. drug-induced) urinary retention

요실금 종류	정의	특징
복압 요실금	신체 운동이나 기침, 재채기를 할 때 복압이 상승하며 발생하는 요실금	요실금의 가장 흔한 유형 주요 원인: 출산으로 인한 신체적 손상, 임신 기간과 폐경기의 호르몬 변화, 복부에 힘을 주는 만성적인 기침이나 변비, 비뇨생식기계의 수술 과거력 등
절박 요실금	방광의 소변량과 무관하게 소변이 마려운 느낌이 갑자기 생긴 후 발생하는 요실금	발생기전: 대부분의 경우 방광벽에 있는 방광배뇨근의 불안정과 과민성으로 발생 원인: 대개 규명이 어렵다. 신경 질환( 뇌경색, 다발성 경화증), 신경 손상, 급만성 요로 감염이나 방광암, 방광 결석 등
혼합(복합성) 요실금	복압 요실금과 절박 요실금이 혼재되어 나타나는 요실금	절박 요실금 증상을 더 크게 호소하므로 절박 요실금을 먼저 치료하는 경향
범람 요실금	소변이 방광에 가득차서 넘쳐흐르는 상태로 소변을 찔끔찔끔 보면서 완전한 배뇨가 되지 않는 요실금	원인: 방광배뇨근의 수축장애, 요로폐색
기능성 요실금	신체적 또는 정신적 장애로 제때에 화장실에 갈 수 없을 때 발생하는 요실금	인지기능의 저하나 거동장애 환자에서 동반가능

기타 요실금
- 지속성 요실금(continuous urinary incontinence): 한 순간만이 아니라 지속적으로 소변이 누출되는 것
- 웃음 요실금: 청소년기의 소녀들에서 살짝 웃을 때 발생하는 요실금
- 성행위 중 발생하는 요실금 등

- 물리치료: 골반저근훈련[PFMT: pelvic floor muscle training], 전기자극치료 (표면전극), 자기장치료, 후경골신경자극[PTNS: Posterior (percutaneous) tibial nerve stimulation]
