최근 HCV의 증식과정과 생활사가 밝혀지면서 바이러스의 증식에 필수적인 단백질을 표적으로 하는 약물이 개발되었다.

작용 기전 및 분류

DAA는 HCV 생활사에 직접 작용하여 항바이러스 효과를 나타낸다.
작용 기전과 표적 부위를 기준으로 분류
1. HCV NS3/4A 단백분해효소억제제(protease inhibitor, PI)
2. NS5A 억제제(NS5A inhibitor)
3. NS5B 중합효소억제제(NS5B polymerase inhibitor)

NS3/4A protease inhibitor

가장 먼저 개발된 DAA
HCV 증식에 필수적인 다단백 분해과정을 차단

1세대: boceprevir, telaprevir
2세대: simeprevir, asunaprevir, paritaprevir, grazoprevir, voxilaprevir, glecaprevir

NS5A 억제제

HCV 복제 및 조립 억제
여러 유전자형에 작용하고 다른 약제와 병합할 경우 상승 효과를 나타낸다.

NS5B 중합효소억제제

구조에 따라 2가지로 분류

위 그림은 바이러스의 복제과정을 나타내고 있으며, 각 복제단계마다 어떠한 DAA 약제가 작용하는지를 보여주고 있다.
HCV는 바이러스 효소(NS2/3와 NS3/4A 단백분해효소)와 세포 신호 펩티드분해효소에 의하여 10개 부분의 구조단백질 및 비구조단백질 부분으로 구성되어 있는데, 바이러스 증식에는 비구조단백질인 NS3, NS4A, NS4B, NS5A와 NS5B 등이 관여한다.
1. 단백분해효소 억제제(protease inhibitors): HCV는 RNA 바이러스인데 복제의 시작 단계에서 단백중 합체(protein polymer)가 만들어진다. 복제가 진행되려면 거대단백(polyprotein)이 각각의 기능단위인 효소로 쪼개어져야 하는데 이 과정에서 단백분해효소가 필요하다. 이 단계를 억제하는 약제가 바로 단백분해효소 억제제이다.
2. NS5A 억제제(NS5A inhibitors): 바이러스 증식에 관여하는 비구조단백질 중 하나인 NS5A에 작용하는 약제
3. NS5B 중합효소 억제제(polymerase inhibitors): NS5B단백은 RNA 복제에서 가장 중요한 중합효소로서 NS5B 중합효소 억제제는 RNA 의존 중합효소를 억제하여 효과를 발휘. Nucleos(t)ide, non-nucleos(t)ide 계열로 분류. Nucleos(t)ide 계열의 경우 연쇄 종료를 유발하는 자연적인 중합효소와 유사하여 RNA 사슬 내로 합일화되어 직접적으로 사슬을 중단시켜버림으로써 바이러스 증식을 억제하고 non-nucleos(t)ide 계열의 경우에는 중합효소의 활성 부위와 결합하는 기질로 사용되어 중합효소를 억제한다.
