치료원칙

언제 어떻게 치료하나?

ITP로 진단되었다고 해서 모든 경우 치료하는 것은 아닙니다. ITP의 치료 목표는 혈소판 수를 normal range로 회복시키고 유지하는 것이 아니기 때문입니다.

ITP의 치료는 severe thrombocytopenia로 인한 major bleeding 혹은 life hreatening bleeding을 얘방할 수 있는 안전한 혈소판 수(safe platelet count)를 유지하는 데 초점을 맞추게 됩니다. (∵ 혈소판 수가 30,000/μL 이상인 경우에는 저혈소판증으로 인한 사망의 위험은 높지 않음)

치료 방법

항체가 결합된 혈소판의 세망내피 섭취를 감소시키는 약제 사용
항체 생산을 억제하는 약제 사용
혈소판 생성을 증가시키는 약제를 병합 또는 단독 사용

혈소판 감소 ≤30,000/μL 입원을 필요로 하는 심각한 출혈증상(혹은 출혈예고증상)은 없는 경우	상당한 출혈 증상(significant bleeding symptoms)이 있는 경우 심한 혈소판 감소(severe thrombocytopenia): ≤5,000/μL 임박 출혈 징후(signs of impending bleeding): 예. retinal hemorrhage, large oral mucosal hemorrhages

단일 제제 투여 corticosteroid: prednisone 1mg/kg	복합 투여(combined-modality therapy) high dose steroid + (IVIG or anti-RhD) ± 면역억제제

입원 치료가 필요한 경우에는 corticosteroid와 더불어 IVIG(혹은 anti-RhD)를 같이 투여하게 됩니다. 이는 IVIG의 치료효과가 신속하기 때문입니다(24시간 이내 효과 발현)

1차 치료 방법

ITP의 1차 치료

Anti-D	50-75ug/kg, 단회 투여 Initial rates: 성인:80%; 소아: 50%-80%; time: 24-48 시간; durability: typically 3-4 wk based on antibody half-life

스테로이드는 성인 ITP 환자의 치료제로 가장 많이 사용하는 약제

<참고: immunoglobulin 대량 투여>

혈소판에 자가항체가 결합되면 대식세포에 존재하는 Fc 수용체가 그 항체의 Fc segment를 인식하여 항체와 함께 혈소판을 탐식한다.
immunoglobulin을 대량으로 점적 정맥주사할 경우 대식세포의 Fc 수용체가 immunoglobulin에 의해 포화되기 때문에 혈소판에 결합한 자가항체를 인식할 수 없게 되어 혈소판은 대식세포에 의해 탐식되지 않는다.

2차 이상의 치료

2차 이상 치료 원칙

스테로이드 치료에 실패한 lTP 환자에 사용 가능, 환자의 출혈 정도, 동반 질환, 환자의 기대와 순응도 등을 고려하여 치료 방법 결정
2차 치료: 비장절제술, 그 외 여러 종류의 약제

2차 치료 선택 지침: 성인/소아

	소아	성인
Splenectomy	상당하고 지속적인 출혈이 있고 다른 치료(corticosteroids, IVIg, anti-D) 불응하는 소아에서 추천된다.	스테로이드 치료에 실패한 경우 추천된다.
Rituximab	다음과 같은 경우 고려될 수 있다. 기존의 치료를 했음에도 현재 상당한 출혈이 발생하는 경우 혹은 삶의 질의 향상을 원하는 경우 만성ITP에서 다음과 같은 경우 Rituximab를 고려할 수 있다. Splenectomy을 대체요법으로 고려 Splenectomy을 실패한 경우 치료로 고려	1차 치료(corticosteroids, IVIg, Splenectomy)에 실패하였고 출혈의 위험성을 가지고 있는 성인에서 고려할 수 있다.
Thrombopoietin Receptor Agonists	소아에 대한 권고안이 없음	다음과 같은 경우에다 출혈의 위험성을 가지고 있는 시행 비장절제술 후 재발한 경우 비장절제술의 금기인 경우 적어도 1가지 이상 다른 치료에 실패한 경우 1차 치료(corticosteroids, IVIg)에 실패하였고 비장절제술을 받지 않았는데 출혈의 위험성을 가지고 있는 성인에서 고려할 수 있다.

2차 이상 치료 방법

Splenectomy

수술하기 전 encapsulated organism(pneumococcus, meningococcus, Hemophilus influenzae)에 대한 vaccination을 시행해야 한다.

Thrombopoietin(TPO) receptor agonist

현재 2가지 종류 존재: Eltrombopag, Romiplostim
