신산통의 치료 시 권고되는 약물

우선순위	치료방법
1차 선택	NSAID 투여: Diclophenac sodium, Indomethacin, Ibuprofen
2차 선택	Hydromorphine, Pentazocine, Tramadol
기타	α-blockers (recurrent colics 감소목적)

신산통의 치료를 포함한 신결석(요로결석)의 급성기 치료는 다음과 같이 정리할 수 있다.

결석 배출 요법(Stone relief)

적극적인 결석제거의 적응증(요도 결석의 경우)	적극적인 결석제거의 적응증(신결석의 경우)
자연배출 될 가능성이 낮은 결석 적절한 약물투여에서 통증이 지속되는 경우 지속적인 폐쇄가 있는 경우 신기능부전(renal insufficiency)이 있는 경우: 신부전, 양측폐쇄, 단일신장	결석의 크기가 커지는 경우 결석이 잘 형성되는 환자(고위험군)에서 발생한 결석 결석에 의해 페쇄가 발생한 경우 감염이 동반된 경우 증상을 동반한 결석인 경우: 통증, 혈뇨 결석 >15 mm 결석 <15 mm 이지만 경과관찰이 적응되지 않는 경우 환자가 결석제거를 원하는 경우 2-3년 이상 결석이 지속되는 경우

보존적 치료: 통증조절, 경구 또는 정맥을 통한 충분한 수분 공급, 이뇨제 투여 등 약물치료 약물을 이용한 배출촉진요법: 알파차단제(alpha-blocker), 칼슘통로 차단제, 코르티코스테로이드(corticosteroid), 비스테로이드소염제 등 약물을 이용한 용해요법 경구화학용해술 경피화학용해술 체외충격파쇄석술	경요도결석제거술 경피신결석제거술 복강경 및 로봇을 이용한 결석제거술
