간 손상정도 분류표

단계	손상 정도
I	혈종: 피막하, 표면적 10% 이하 열상: 피막손상, 실질 깊이 1 cm 이하
II	혈종: 피막하, 표면적 10-50%; 실질 내 반경 10cm 이하 열상: 실질 깊이 1-3 cm, 10cm 이하의 길이
III	혈종: 피막하, 표면적 50% 이상이거나 증가하는 경우; 실질 내 혹은 피막하 혈종이 파열된 경우; 실질 내 반경 10 cm 이상이거나 증가하는 경우 열상: 실질 깊이 3 cm 이상
IV	열상: 실질파열이 간의 25-75%를 차지하거나 한 엽의 Couinaud 분절이 3개 이하 손상된 경우
V	열상: 실질파열이 간의 75% 이상이거나 한 엽의 Couinaud 분절이 3개 이상 손상된 경우 혈관성: 간주변 정맥의 손상(예: 간후방대정맥, 간정맥)
VI	혈관성: 간박리(avulsion)

간 손상정도는 6단계로 나눌 수 있다. 혈종은 열상이나 혈관성으로도 쉽게 발생할 수 있으므로 어느 단계에서나 발생이 가능하다고 볼 수 있다. 따라서 혈관 손상이 있는 경우가 Grade V 이상이라고 보면 쉽게 이해할 수 있다. 그리고 열상 깊이나 정도에 따라서 Grade I/II/III 은 실질의 깊이 1 cm이하/1-3 cm/3 cm이상으로 구분 하면 된다. 그리고 열상이 심해서 실질파열이 1/4에서 3/4를 차지한다면 Grade IV이다. Grade IV이상의 경우 수술이 반드시 필요하다.

평가 및 치료

환자의 혈역학적 소견에 이상이 없다면 보존적인 치료 시도(간은 스스로 치유되는 능력이 높으며 재생력이 뛰어나기 때문)
반드시 중환자실에 입원되어야 하며 지속적인 혈역학적 감시와 혈액검사가 필요하며 필요시 영상 추적검사 시행

둔상에 의해 간손상이 발생했을 경우에는 가능하면 보존적 치료를 시도하므로 꼭 수술을 시행하는 경우를 알아두면 되겠다.

비수술 치료

미동부외상학회에서는 2012년 복부 둔상에 의한 간손상환자에서 비수술적인 치료에 대한 지침을 발표하였으며, 요약하면 다음 표와 같다.

Selective nonoperative management guideline for blunt liver injury

Level I :

Indications of urgent laparotomy after blunt abdominal injury
1. Patients who are hemodynamically unstable
2. Patients who have diffuse peritonitis

Level II :

Hemodynamically stable patients without peritonitis with an isolated blunt hepatic injury – no indication for routine laparotomy
Abdominal CT scan with IV contrast : to identify and assess the severity of injury to the liver in the hemodynamically stable blunt abdominal trauma patient without peritonitis
Angiography with embolization: a first-line intervention for a patient who is a transient responder to resuscitation as an adjunct to potential operative intervention.
No absolute contraindications of nonoperative management in stable patients : severity of hepatic injury (as suggested by CT grade or degree of hemoperitoneum), neurologic status, age of more than 55 years, and/or the presence of associated injuries
Angiography with embolization : in a hemodynamically stable patient with evidence of active extravasation (a contrast blush) on abdominal CT scan.
Nonoperative management of hepatic injuries should only be considered in an environment that provides capabilities for monitoring, serial clinical evaluations, and an operating room available for urgent laparotomy.
