세로토닌 증후군이란?

세로토닌의 활성을 증가시키는 선택적 세로토닌 재흡수 억제제(selective serotonin reuptake inhibitor, SSRI), 삼환계 항우울제(tricyclic antidepressant, TCA), 모노아민산화효소억제제(monoamine oxidase inhibitors, MAOI) 등 세로토닌계에 영향을 주는 약물이나 이들 약물 간의 상호작용에 의해 세로토닌 수용체가 과도한 자극을 받아 발생

대부분 SSRI 과다 복용에 의해 발생
cf. 세로토닌: 식욕, 수면, 기분, 행동, 체온 조절, 심혈관 기능, 근육 수축 등 다양한 기능에 관여하는 신경전달물질

임상 양상

의식 상태 변화, 자율신경의 과다 반응에 의한 심박동 수 증가, 혈압 상승, 떨림, 발한, 동공 확장 등의 증후, 신경근 이상
비교적 단기간 내에 발생. 세로토닌을 증가시키는 약물 투여 후 50%는 2시간 이내에, 25%는 24시간 이후에 증상이 발생하며, 드물게는 수 주 후에도 발생

진단에 특이적인 혈청학적, 영상 검사법은 현재까지 없음
세로토닌계에 영향을 미치는 약물의 사용력이 있으며, 다른 이유로 설명되지 않는 의식 상태의 변화, 자율신경의 과다 반응, 신경근 이상 같은 임상 양상이 있을 때 세로토닌증후군 진단 가능

Hunter’s Decision Rules

5가지 기준 중 지난 5주 간 세로토닌 항진 약물을 복용한 환자에서 1개 이상이 발생한 경우 세로토닌 증후군으로 진단 가능

세로토닌증후군이 의심되면 즉시 약물 투여 중단
유발 약물의 중단 및 증상에 대한 대증 요법 시행
유발 약물 중단: 약물 중단 후 24시간 내 대부분 증상이 호전되나, 긴 반감기를 가진 약물 또는 배설 장애가 있는 경우에는 더 오래 지속 가능
증상에 대한 치료; 벤조다이아제핀, 세로토닌길항제 등
