발목-위팔지수(Ankle-brachial index, ABI)란? 검사방법 결과 해석

발목-위팔지수(Ankle-brachial index, ABI)란?

하지 말초동맥질환의 선별검사와 진단에 가장 널리 쓰이는 검사방법. 매우 간단하고 비침습적인 진단 방법
하지 동맥 폐쇄의 정도를 보기 위한 지표
발목과 상완에서 혈압을 측정한 후 발목의 수축기혈압을 상완의 수축기혈압으로 나눈 값

발목 상완 지수(Ankle brachial index, ABI)를 측정하는 것은 전자 혈압계를 비롯한 혈압을 확인할 수 있는 비교적 간단한 장비만 있으면 일차 진료실에서 쉽고 간편하게 시행할 수 있어 PAD를 진단하는데 있어서 가장 적절한 선별검사 방법이다.

검사방법

누운 자세에서 도플러를 이용하여 양쪽 위팔(brachial arteries)과 양쪽 발목(dorsalis pedis와 posterior tibial arteries)에서 혈압 측정

당뇨병환자나 고령에서처럼 혈관 벽에 칼슘이 침착되어 혈관의 탄력성이 떨어지면 혈압을 측정할 때 혈압측정띠가 혈관을 제대로 압박하지 못하여 원래보다 높은 수치가 나올 수 있습니다. 이런 경우에는 발가락에서 혈압을 측정하여 발가락-상완지수(toe-brachial index)를 구하거나, 이중초음파검사를 하여 도플러파형을 함께 분석합니다.

결과 해석

적절한 ABI의 기준치에 대해서는 참고문헌에 따라 다소 다르긴 하나 현재까지 PAD를 진단할 때 일반적으로 0.9라는 기준치를 사용하고 있다.

ABI 0.9를 진단 기준으로 했을 때 특이도의 경우 64-99%, 민감도의 경우 68-95%까지 다양하게 보고되고 있다.

말초혈관질환 진단을 위해서 휴식기 발목-위팔지수(Ankle-brachial index, ABI)를 가장 먼저 실시할 것을 권고
cf. 말초혈관질환의 고위험군이나 병력 청취나 진찰 과정에서 의심할 만한 정황이 없다면 발목-위팔지수를 검사해 보는 것을 권고
cf. 말초혈관질환의 고위험군이 아니고 병력 청취나 진찰 과정에서도 의심할 만한 정황도 없다면 발목-위팔지수 검사를 권고하지는 않음

휴식기 발목-위팔지수 결과는 비정상(≤ 0.90), 경계(0.91-0.99), 정상(1.00-1.40), 압축이 되지 않는 혈관(>1.40)으로 보고
