Lung nodule에 관한 guideline이 생각보다 종류가 많습니다. 영상의학과 관련 학회에서 쓴 것도 있고 호흡기내과에서 낸 기준들도 있고 다양합니다.
해리슨 내과학의 경우 해리슨 18판에서 19판으로 넘어오면서 SPN의 접근에 관한 내용에 크게 변경되었습니다(20판은 19판과 내용 동일). 19판에는 2013년 ACCP (American College of Chest Physicians)에서 내놓은 SPN 관련 가이드라인에 있는 내용을 토대로 하고 있습니다. 해리슨 18판, Fleischner Society에서 내놓은 2005년 statement에 보면 pulmonary nodule을 특별히 세부적으로 분류하지 않고 접근법을 제시하고 있습니다. 하지만 Fleischner Society에서 내놓은 2013년 statement, 미국 흉부학회(American College of Chest Physicians)에서 발표된 가이드라인 3판(2013년)를 보면 pulmonary nodule을 Solid nodule, nonsolid nodule (pure GGN), subsolid nodules (mixed GGN)로 나누어서 다른 접근법을 제시하고 있습니다.
해리슨 19판, 20판은 2013년의 최신 가이드라인을 바탕으로 하지만 Fleischner Society 2005년 statement도 알고리즘에 포함시키고 있습니다.
참고로 2017년 fleischner society guideline이 업데이트되었습니다. 해리슨 20판(2018)은 이를 반영하지 않았습니다.

2017 Fleischne 가이드라인에서는 nodylar type에 따라 나누고 solid nodule인 경우 nodule 크기, pulmonary nodule의 malignancy risk에 따라 후속조치 기준을 세분화하였다.

malignancy risk를 분류할 때 American College of Chest Physicians (ACCP)에 제안한 분류 카테고리를 따르라고 제안하고 있음. ACCP 가이드라인에서는 malignancy risk에 따라 low, intermediate, high로 나누고 있다.

반면 2017 Fleischne 가이드라인에서는 malignancy risk를 low인지, high인지로 나누고 있다. 해당 가이드라인에서는 ACCP 가이드라인 기준 intermediate risk와 high risk를 합쳐 high risk로 예측하라고 권고하고 있다.[To estimate high risk, we recommend combining the ACCP intermediate-risk (5–65% risk) and high-risk (>65% risk) categories]
