골수증식종양(myeloproliferative neoplasia, MPN)

골수증식종양은 한 종류 이상의 골수세포의 증식을 특징으로 하는 클론성 조혈세포질환
WHO 혈액종양 분류법이 나오기 전까지 골수증식성질환 (MPD : Myeloproliferative disease)으로 불렸던 질환군
질환의 발생 초기에는 골수의 세포충실도 증가를 보이며 효과적인 조혈세포의 성숙과 말초혈액에서의 골수구, 적혈구, 혈소판 등의 증가를 동반하는 것이 특징이다.
서서히 진행하지만 결국 골수섬유화로 인한 골수부전이나 급성골수성백혈병으로 이행하여 사망한다.

MPD는 다능조혈전구세포 (Multipotent hematopoietic progenitor cell)의 질환이다.

하나 이상의 혈구 세포로 과증식을 보이고, 주요한 이형성 (dysplasia)은 없다. 골수외 조혈(Extramedullary hematopoiesis)과 골수섬유화 (Myelofibrosis)가 흔한 소견이고, 급성백혈병 (Acute leukemia)로의 전환도 가능하다.

골수증식종양에서 중요한 질환은 크게 4가지입니다.
1. polycythemia vera(PV)
2. primary myelofibrosis(PMF)
3. essential thrombocytosis(ET)
4. chronic myelocytic leukemia(CML)

CML은 만성백혈병 파트에서 다루고 여기에서는 PV, PMF, ET에 대해서만 다루겠습니다.

분류

증식하는 세포의 종류에 따라 다양한 아형으로 분류

WHO 분류

이전에는 BCR-ABL1 양성 만성골수성백혈병을 제외한 다른 아형을 진단할 경우 각 아형을 구분하기 위해서 뿐만 아니라 세포의 반응성 증식과 클론성 증식을 구분하기 위해 다양한 진단기준이 필요했다.
2008년에 제시된 진단기준에서는 JAK2와 같은 유전자변이와 함께 거핵구의 형태와 분포 등 형태학적 특징에 대한 기준이 제시되어 진단이 한결 쉬워졌다.

‘필라델피아염색체 음성’인 전형적인 골수증식종양(philadelphia chromosome-negative classical MPN): 진성적혈구증가증, 본태혈소판증가증, 일차골수섬유증(만성 골수백혈병 제외)
