Non renal loss : GI loss (Diarrhea), Sweat
Renal loss (Uk >20mEq/day, 15mEq/L 단 polyuria가 아닐 때)
1. Increased distal flow and distal Na delivery : osmotic diuresis
2. Increased distal potassium secretion
  Aldosterone (like effect) excess : ECFV expansion (hypertension;ⅰⅱⅲⅳ), ECFV contraction (normotension; ⅴⅵ)
  1. Primary hyperaldosteronism (adrenal adenomas, hyperplasia, carcinoma)
  2. Secondary hyperaldosteronism (malignant hypertension, renin-secreting tumors, renal artery stenosis)
  3. Cushing’s syndrome
  4. Liddle’s syndrome
  5. Diuretics / Bartter’s syndrome, Gitelman’s syndrome
    ECFV contraction 에 의한 aldosterone증가 뿐 아니라 distal Na delivery 증가로 인해 hypokalemia 발생.
  6. Vomiting/Nasogastric suction
    = 구토 시 배출되는 위액에는 칼륨이 매우 적지만, 구토로 인한 ECFV contraction 때문에 Aldosterone이 증가해서 hypokalemia와 metabolic alkalosis가 발생합니다. 또한 구토에 의한 metabolic alkalosis는 collecting duct로 이동하는 HCO3-를 증가(nonreabsorbed anions)시켜서 hypokalemia를 유발합니다. 설사와 구토 모도 저칼륨혈증을 유발하지만 유발하는 기전은 다릅니다. 설사는 칼륨을 장으로부터 직접적으로 배출하여(GI loss) 저칼륨혈증을 유발하지만 구토는 위로부터 칼륨 배출이 일어나는 것이 아니라 콩팥을 통한 칼륨의 소실이(renal loss) 주된 원인입니다.
    Diarrhea : hypokalemia, Metabolic Acidosis
    Vomiting : hypokalemia, Metabolic Alkalosis
    Distal delivery of nonreabsorbed anions
  7. HCO3- : Proximal renal tubular acidosis
  8. Other than HCO3- : penicillin derivatives (penicillin, nafcillin, dicloxacillin, ticarcillin, oxacillin, and carbenicillin)
3. Magnesium Deficiency
