병태 생리
- Plasma cell에서 secretory IgA 생산의 증가
- 간과 콩팥의 mesangium에서 IgA 청소율이 저하
→ IgA가 Mesangium에 침착하면서 형성된 Immune complex에 의한 토리콩팥염 발생
Cf. Henoch Scholein Purpura (HSP)
- HSP는 혈관에 IgA 침착되어 발생하는 전신적인 혈관염. HSP에 의해 IgA 신증이 나타날 수도 있음
- 혈관염이므로 IgA신증과 달리 전신적인 증상이 나타남.
진단
- 역학 : 원발성 사구체 질환 중에서 가장 흔함, 남자에서 흔함, 10-20대 peak로 새로 발생, 인종적 차이 있음
임상증상
IgA신증환자는 주로 두 가지 임상양상으로 내원함
- Recurrent episodic gorss(macroscopic) hematuria after or during upper respiratory infection: 선홍색, 흔히 pharyngitis, GI infection, vaccination, 격렬한 운동 24-48시간 후에 나타남.또는
- Persistent asymptomatic microscopic hematuria: 무증상성 소변이상(AUA)
DDx. PSGN - 상기도 감염 이후 발생 : IgAN 에 비해 긴 잠복기
- 육안적 혈뇨 : 콜라색
- 반복적으로 발생하지 않음
- 고혈압, 부종같은 동반 증상 있음
DDx. AUA - TBM : 예후 좋음
- Alports Synd : 감각신경성 난청,전방원추수정체 동반. 예후 나쁨

검사 소견
- 혈액 검사 : 20-50%환자에서 IgA 가 증가.
- 소변검사 : 단백뇨 (질환 초기에 동반되기도 하지만 대부분 질환이 경과 중 늦게 나타남.)
병리 소견
확진을 위해서는 콩팥 생검을 반드시 해야 함.
LM : global or segmental mesangial hypercellularity with some sclerosis and tublointerestitial fibrosis
IF : mesangial IgA deposition
EM : mesangial electron dense deposit.
- IgA가 mesangium에 침착되는 질환
- chronic liver disease, Crohn’ s disease, adenocarcinoma
- dermatitis herpetifonriis, mycosis fiongoides, leprosy
- ankylosing spondylitis, relapsing polychondritis, Sjogrens syndrome
- Chronic bronchiectasis, idiopathic interstitial pneumonia
치료 예후
치료
- 대부분 치료 필요 없음
단, 콩팥기능이 저하되거나 단백뇨가 있을 때, ACEi 를 쓰는 것이 도움이 될 수 있으며, RPGN 양상으로 나타난 경우 Steroid, Cytotoxic agent, plasmapheresis를 할 수 있음
예후
- 혈뇨가 지속되더라도 콩팥기능은 보존되어 투석이 필요한 말기신부전으로 진행하는 환자가 적음
(Progression to renal failure : 25~30% over 20~25yrs)
- 나쁜 예후 인자 (콩팥기능 저하진행 가능성을 예측할 수 있는 인자)
- Hypertension
- Persistent(longer than 6 month) Proteinuria : m/i
- Absence of episodes of macroscopic hematuria
- Male
- Older age of onset reduced renal function at diagnosis
- Biopsy: glomerulosclerosis or interstital fibrosis
