병태생리

SLE의 합병증으로 생기는 사구체 신염으로 SLE 환자 중 약 60%이상에서 동반
circulating immune complex가 사구체에 침착 → complement activation되면서 염증 발생

Lupus nephritis이외에도 complement가 활성화되어 발생하는 사구체 신염으로는(활성화 되니까 혈청에서 complement level 은 떨어집니다) MPGN, PSGN 이 있고 이 세 질환을 구별해야 합니다.

Antigen-antibody complex 에 의해 complement가 활성화되는 classic pathway에서는 C3, C4가 모두 감소합니다.

그리고 면역형광염색에서 IgG, IgA, IgM, C3, C1q에 모두 양성을 보여서 "full house pattern"이라고도 한답니다.

진단

일반적으로 콩팥생검은 소변검사에서 단백뇨가 나오거나 혈액검사에서 혈청 크레아티닌이 상승할 경우에 시행

lupus환자에서 소변, 혈액검사가 정상이라도 lupus nephritis 가 있을 수 있습니다. 하지만 단백뇨가 나오거나 질소혈증이 있을 때, lupus 신염에 대해 치료하기 때문에 모든 lupus환자에서 콩팥 생검을 하는 건 아닙니다.

Class IV	Diffuse nephritis	Diffuse endocapillary ± extracapillary proliferation with diffuse subendothelial immune deposits and mesangial alterations
Class V	Membranous nephritis	Thickened basement membranes with diffuse subepithelial immune deposits; may occur with class III or IV lesions and is sometimes called mixed membranous and proliferative nephritis
Class VI	Sclerotic nephritis	Global sclerosis of nearly all glomerular capillaries

콩팥 생검을 하고 나온 콩팥 조직검사결과에 따른 분류입니다. 이 분류는 임상과 일치도가 높을 뿐 아니라 분류에 따라 치료를 결정하고 예후를 예측할 수 있어 임상에서 매우 중요한 분류입니다. 주의할 사항은 처음 조직검사에서 나온 결과가 계속 유지 되는 것이 아니라 병의 경과에 따라 class가 전환되기로 하므로 사구체신염이 악화나 재발 시에는 조직검사를 다시 해야 합니다.

임상양상
- Class III-IV는 주로 acute nephritic Syndrome으로 나타남
- Class V는 주로 nephrotic syndrome으로 나타남
- Class VI는 CGN으로 나타남

치료와 예후

Class I, II : 치료 필요 없음
Class III (severe), IV, V (mixed membranous)
high dose steroid + Cyclophosphamide or MMF 2-6개월 → 이후 lower dose steroid + MMF or AZA
- 치료하지 않으면 Class IV가 가장 예후가 나쁨. 단, 치료를 통해 관해(콩팥기능 정상, 단백뇨 <330mg/day) 오면 예후 매우 좋음.
Class VI 혈액투석 또는 신장이식
- Lupus nephritis 환자의 20%가 end stage renal disease
- 콩팥기능 부전이 올 경우, lupus 활성도는 감소(uremia에 의한 면역억제로 인해)
