※ 임계 하지 허혈(critical limb ischemia)
- 허혈이 심해져 휴식기 통증(rest pain)과 함께 다리가 창백하거나 청색증을 보이며 하지에 궤양(ulceration)과 괴저(gangrene)와 같은 조직 소실이 임박한 상황
Chronic limb ischemia의 증상
주요 증상
- 통증
- 간헐적 파행(intermittent claudication) - "전보다 조금 덜 걸어도 다리가 아프다" "종아리가 당긴다, 조인다, 터지는 듯하다"
- 휴식기 통증(rest pain)
- Parasthesia, Pallor, 발의 맥박이 감소하거나 소실
- "발이 차다/시리다" "추운 곳에서 발 색깔이 변한다(하얗게, 혹은 붉거나 보라색), 저리다"
- 피부변화: 얇고 건조하며 털이 없어지고 창백하며 차가운 피부, 피하지방 소실, 발톱이 두꺼워지고 부스러지는 현상, 근육위축, ulcer or gangrene - "상처가 잘 낫지 않는다"
간헐적 파행(intermittent claudication)
- 부위: 종아리(m/c), 대퇴부, 둔부, 발 등

진단
임상병력 청취상 전형적인 파행(claudication), 관절과 연관 없는 움직일 때의 사지 증상(전형적인 파행 증상은 아님), 보행기능 저하, 휴식기 허혈 통증 등이 있으면 말초혈관질환 의심 가능

Aortoiliac occlusive disease(AIOD, Leriche syndrome)
개요
정의
- thromboembolic obliteration of abdominal aorta & iliac artery, aortic bifurcation
분류
- TASC(Trans-Atlantic Inter-Society Consensus) classification of aorto-iliac lesions
Class
Type A
- Unilateral or bilateral stenosis of CIA
- Unilateral or bilateral single short (3 cm) stenosis of EIA
Type B
- Short (<3 cm) stenosis of infrarenal aorta
- Unilateral CIA occlusion
- Single or multiple stenoses totaling 3–10 cm involving the EIA not extending into the FA
- Unilateral EIA occlusion not involving the origins of internal iliac or FA
Type C
- Bilateral CIA occlusions
- Bilateral EIA stenoses 3–10 cm long not extending into the FA
- Unilateral EIA stenosis extending into the FA
- Unilateral EIA occlusion that involves the origins of internal iliac and/or FA
- Heavily calcified unilateral EIA occlusion with or without involvement of origins of internal iliac and/or FA
Type D
- Infrarenal aortoiliac occlusion
- Diffuse disease involving the aorta and both iliac arteries requiring treatment
- Diffuse multiple stenoses involving the unilateral CIA, EIA, and FA
- Unilateral occlusions of both CIA and EIA
- Bilateral occlusions of EIA
- Iliac stenosis in patients with AAA requiring treatment and not amenable to endograft placement or other lesions requiring open aortic or iliac surgery

Thromboangitis obliterans(Buerger’s disease, 폐쇄혈관혈전염)
개요
- 상, 하지의 small and medium sized arteries and veins을 침범하여 occlusion을 유발하는 염증성 폐쇄 혈관 질환(inflammatory occlusive vascular disease) →일종의 혈관염(vasculitis)
- 습관적 흡연자인 20~40대의 남자
- 하지의 혈관이 폐쇄되어 자발성 괴사를 일으킴. 동맥과 동시에 부근 정맥까지도 염증을 일으켜 폐쇄
진단
임상양상
- Ischemic rest pain, claudication
- Acral skin discoloration/coldness, Raynaud’s phenomenon
- migratory superficial phlebitis / Thrombophlebitic nodules or strings(often migratory)
- Subungual and skin infection, phlegmon
- Gangrene, Acral ulcer

- 흡연력
- 50세 이전의 발병
- 슬와부 하방의 동맥 폐쇄(Infrapopliteal occlusion)
- 이행성 정맥염(phlebitis migrans) 또는 상지의 병변
5 흡연력 이외의 죽상경화증의 위험인자가 없을 것 이상의 다섯 가지 조건이 다 충족되는 경우만 진단 | - Age of onset less than 45 years
- Current or recent history of tobacco use
- Presence of distal extremity ischemia with claudication, rest pain, ulcers, or gangrene
- Exclusion of autoimmune disease, hypercoagulable states, diabetes mellitus, and proximal source of embolism
- Consistent arteriographic findings in the clinically affected and unaffected limbs
