Cholangiocarcinomas distal bile duct cancer vs distal CBC cancer

담관암을 보다도면 다음과 같은 용어를 접할 수 있는데, 개념이 약간혼돈스러운 부분이 있어 개념정리를 해보았다.

Klatskin tumors
CBD cancer
distal CBD cancer(원위부 담관암)
distal biliary cancer

흔히 mid CBD, distal CBD라는 표현으로 common bile duct의 중간 부분, 아랫부분을 지칭하며 사용되곤 한다.

Rt. hepatic duct와 Lt. hepatic duct로 나뉘어지는 부분부터 common hepatic duct이고 cystic duct와 만나 common bile duct(CBD)가 됩니다.

Extrahepatic bile duct는 perihilar part와 distal part로 나뉘고 perihilar part는 common hepatic duct와 Rt. and Lt hepatic duct이며 distal part는 common bile duct이다.

Cholangiocarcinomas

Cholangiocarcinomas는 intra- or extrahepatic bile duct epithelium에서 기원하는 종양이다. 즉 bile duct cancer의 또 다른 이름이다.

distal bile duct cancer vs distal CBC cancer

임상적으로 bile duct cancer(cholangiocarcinoma)는 다음과 같이 분류

Intrahepatic bile duct cancer
extrahepatic bile duct cancer
- Perihilar (혹은 hilar) bile duct cancer = Klatskin tumors
- mid bile duct cancer
- Distal bile duct cancer = Common bile duct cancer

즉 distal biliary cancer, distal bile duct caner, distal cholangiocarcinoma는 common bile duct cancer(CBD cancer) 자체를 의미한다.
distal biliary cancer
= distal bile duct caner
= distal cholangiocarcinoma
= common bile duct cancer(CBD cancer)

해부학적으로 distal CBD cancer를 의미하지 않는다.
- distal CBD(CBD의 distal)와 distal bile duct(CBD)는 다른 부위이다.

Distal bile duct cancer는 common bile duct cancer를 의미하고 Common bile duct cancer 중에서도 특별히 distal 쪽에 발생하는 distal common bile duct cancer는 periampullary cancer로 따로 분류하기도 한다.
