항암화학요법의 목적 및 방법

항암화학요법은 크게 완치를 목적으로 하는 경우와 완치가 불가능한 경우에 시행하는 경우로 나눌 수 있습니다.

Neoadjuvant chemotherapy :선행(보조)항암화학요법 Adjuvant chemotherapy: 보조 항암화학요법 Induction chemotherapy : 유도 항암화학요법	Palliative chemotherapy(완화요법) Salvage chemotherapy : 1st line chemotherapy 실패한 경우

단독으로 시행가능하지만(primary treatment modality) 완치(cure)를 위한 적극적인 치료 방법으로 사용될 경우 치료 효과를 높이기 위해 수술이나 방사선치료와 함께 사용될 수 있습니다.

항암치료방법	특징	예시
보조 화학요법(adjuvant chemotherapy)	국소 종양을 수술이나 방사선요법으로 치료한 후, 측정할 수는 없지만 남아있는 잔여암세포를 제거하기 위해 사용되는 CTx	Colorectal, Breast, NSCLC, Osteosarcoma
선행 화학요법(neoadjuvant chemotherapy)	종양의 크기가 너무 큰 경우 수술이나 방사선 치료를 시행하기 전에 시행되는 CTx 종양의 크기를 줄여 수술을 가능하도록 한다. 수술 후 organ preservation을 가능하도록 한다. 방사선 치료의 범위를 줄여 부작용을 줄이고 효과적으로 방사선이 전달될 수 있도록 한다.	Organ preservation : Head & Neck cancer, Rectal cancer breast cancer (non-metastatic), Osteosarcoma
유도 화학요법(Induction chemotherapy)	완전관해를 목적으로 시행하는 CTx	Hematologic malignancy
동시 화학요법(concomitant chemotherapy)	국소 종양에 대하여 방사선 치료와 화학요법을 동시에 시행하는 방법	식도암, 폐암, 항문암 등

Chemotherapy의 역할

암마다 chemotherapy가 기여할 수 있는 정도가 다양합니다. 암에 따라 주연으로 활약하는 경우도 있고 조연에 그치는 경우도 있습니다. 아울러 다른 치료법과의 다양한 collaboration 방식이 존재합니다.
