치료원칙 항생제 경험적 항생제 원인병원체별 항생제 처방의 실제

  

치료원칙

  • 농 또는 삼출물이 있는 경우 그람 염색과 세균 배양 검사를 시행하여 원인균을 확인할 수 있으나, 전형적인 병변인 경우 미생물 진단을 위한 검사 없이 치료 가능

 

  • 농가진: 경구 항생제 또는 항생제 연고
    • 농가진에 대한 치료 효과는 항생제 연고와 경구 항생제 간에 차이가 없는 것으로 알려져 있음 
    • 병변 수가 많거나 특정 Streptococcus pyogenes 전파로 사구체신염이 유행하는 경우 경구 항생제 사용이 추천됨 
  • 농창: 경구 항생제

 

경구 항생제를 권장하는 경우

1. 병변 수가 많은 경우

2. 지역사회에 사슬알균감염후 토리콩팥염(poststreptococcal glomerulonephritis)을 유발하는 사슬알균(nephritogenic strains of S. pyogenes)이 유행하는 경우

3. 농창


 

  • 치료 반응이 없는 병변의 경우 MRSA 감염 가능성을 고려하여 경험적으로 doxycycline, trimethoprim/sulfamethoxazole, clindamycin 같은 경구 항생제 사용 가능

  • streptococci와 S. aureus 를 함께 치료할 수 있는 항생제

  • 경구 항생제: amoxicillin/clavulanate, 1세대 cephalosporin, clindamycin
  • 항생제 연고: mupirocin, fusidic acid, retapamulin

경구 항생제는 7일간 사용하며, 항생제 연고는 5일 동안 하루 2회 사용

 

  •  Methicillin-resistant Staphylococcus aureus (MRSA)가 의심되거나 확인된 경우: doxycycline, clindamycin, trimethoprim/sulfamethoxazole

1차 요법

Streptococcal impetigo

  • 병변수가 많지 않는 한 국소항생제는 경구항생제만큼 효과적 
  • With few lesions: Mupirocin ointment or Retapamulin ointment 
                        

Mupirocin ointment 2%

tid  x 5일

Fusidic acid cream 2% 

x 5일

Retapamulin 1% ointment

bid x 5 일

    
  • With many lesions: oral therapy

Penicillin or TMP-SMX DS or Benzathine Penicillin G IM

                        

Penicillin VK

250-500 mg po qid x 5 days 

TMP-SMX DS

1T po bid x 5 days 

Benzathine Penicillin G  

600,000-1,200,000 units IM x one dose


MSSA bullous impetigo

  • 다음과 같이 7일 치료

1. ointment: Mupirocin ointment,  Retapamulin ointment

or

2. PO: Dicloxacillin po or Cephalexin po

 

MRSA bullous impetigo

다음과 같이 7일 치료

1. Mupirocin ointment tid or

2. TMP-SMX DS 1T po bid

(all po regimens): TMP-SMX-DS or (Doxycycline or Minocycline) or (Clindamycin)
