7년 전부터 소화 안 되고 새벽에 쓰리고, 야채 먹으면 속쓰린 증상이 발생했다. 소화제를 달고 산다. 올해부터 더욱 심해졌다.

기본 lab, EGD, abdominal CT
이상 없으면 대장내시경 시행 고려

고령이고 유의미한 weight loss가 있기 때문에 내시경, abdominal CT까지 시행할 필요가 있다.

위내시경, 대장내시경을 한꺼번에 시행할 수도 있겠지만 76세 고령이라 부담스러운 면도 있고, 증상이 주로 상복부관련 증상이라서 단계별 시행전략(위내시경 → 대장내시경)을 택하였다.

abdominal CT(enhance)
1. Focal cortical atrophic change at the interpolar region of right kidney, suggesting post-inflammatory sequelae.
2. A small simple renal cortical cyst in the left kidney.

조직검사 결과: stomach(antrum)
1. Stomach, antrum, endoscopic biopsy(G1); Chronic gastritis, moderate, with foveolar epithelial hyperplasia.
2. Stomach, endoscopic biopsy(G2); Chronic gastritis, moderate, with lymphoid aggregate.
3. Stomach, endoscopic biopsy(G3); Chronic gastritis, moderate, with H. pylori infection.

Helicobacter PyloriI(CLO TEST): Positive

EGD에서 gastric ulcer scar가 관찰되고 있다. 이런 경우 Helicobacter Pylori 균주검사(CLO test)를 시행해야 하는데 이는 H. pylori infection이 궤양 재발의 가장 강력한 위험인자이기 때문이다.

아울러 위 및 십이지장의 소화성궤양(반흔기 포함)이 있는 경우 Helicobacter Pylori 균주검사는 급여인정을 받을 수 있다.

H. pylori infection이 복통의 원인일 수도 있고 아닐 수도 있지만 가능성은 존재하므로 일단 제균치료를 하면서 복통의 호전여부를 지켜볼 필요는 있겠다. 헬리코박터 감염 이외 다른 이상소견은 없기 때문에 추가적으로 대장내시경을 시행해서 체중감소, 다른 위장관 증상을 설명할 만한 기질적인 병변을 탐색해 보고자 한다.
