내원 3개월 전 치매, 폐렴, 전신쇠약 등으로 호흡기내과 입원 후 퇴원하여 현재까지 요양병원 입원 중인 분으로, 2개월 전 L-tube removal 이후 현재 일반죽 식이 중임. 사레걸림 간혹 있고, 딸꾹질이 지속되어 연하검사 위해 보호자 내원

Tongue movement, Bolus formation 및 mastication relatively intact하였고 semisolid, solid intake시, mastication 오랫동안 하여 심한 oral phase delay 소견 보임. early spillage 소견 보임. 이는 지난 검사와 유사한 소견임	initiation delay, laryngeal elevation, pharyngeal contraction의 다소 저하된 소견 보이며, minimal amount의 vallecular residue 확인됨. epiglottic closure 비교적 양호함. multiple swallowing시, residue에 대해 partial clearing 가능함. 이는 모두 지난 검사와 유사한 소견임 지난 검사 시와 마찬가지로 liquid straw에서 PAS 2 penetration 소견 관찰됨. Liquid cup에서도 PAS 2 소견 보임	지난 검사 시와 유사한 양상으로, 이상 소견 없음

일반죽으로 상향 가능합니다. 액체류 섭취 시, cup drinking 시도해볼 수 있으나, Liquid straw, Liquid cup에서 PAS 2 mild penetration 소견 관찰되므로, 사레 걸림 등의 증상 없는지 주의 깊게 살펴보도록 보호자 교육 필요합니다. 또한, vallecular fossa에 minimal amount of residue 관찰되므로, multiple swallowing, effortful swallowing, chin-tuck maneuver과 같은 compensation 반드시 필요합니다. oral phase delay 있으므로 식사 시 지속적 verbal cue 요구됩니다.

- Oromotor fascilitation, Thermotactile stimulation
- Compensation: Multiple swallowing
- Swallowing exercise: Shaker’s exercise
cough, fever, chilling 등의 aspiration pneumonia 증상 발생 여부에 대해 주의 깊게 관찰해야하며 증상 발생할 경우 즉시 병원 내원할 것에 대해 보호자 교육 필요합니다.
3개월 후 VFSS f/u합니다.

보호자 관찰하에 다진반찬(불고기) 등 시도

mastication 여부확인, double swallowing 등 교육, 가래 기침 증가여부관찰 등 교육함

OPD#4	dysphagia 한 달에 1번 정도 발생함 Esophageal Manometry High resolution manometry finding LES Basal (respiratory mean) 76.2 mmHg (13-43) 4s IRP, residual (mean) LES pr 30.6 mmHg (<15) 15회 swallow 시에 60 %에서 LES 정상적으로 이완됨 Body 15회 water swallow 시 60 %에서 정상적인 peristalsis가 관찰됨 식도의 수축 압력은 11 cm/7 cm/3 cm above LES에서 각각 241/169/186 mmHg임(36-134/37-166/41-168) CFV (contractile front velocity) 4.3 (cm/s) (<9.0) DCI (distal contractile integral) 12196 mmHg-cm-s (500-5000) IBP (Intrabolus pressure, @LESR) 14.6 mmHg (<8.4) IBP (Intrabolus pressure, avg max) 17.7 mmHg (<17) UES UES basal pressure 79.6 mmHg (34-104) Peak pharyngeal contraction pressure (@3.0 above mid UES) 309 mmHg UES pharyngeal cordination 정상임 Conclusion) Diffuse esophageal spasm Esophagography Mid-thoracic esophagus에 circumferential multifocal luminal narrowing이 있고, 이로 인해 상부 식도에 contrast의 저류 및 upper esophagus로 reflux를 발생시킴 또한 좁아진 부위는 double contrast study 시행 시에도 insufficient luminal distension을 보임 Mucosal relief view에서는 정상 소견을 보여, mucosal lesion의 가능성은 떨어짐 chest CT에서는 이 부위에 특이 소견 보이지 않고, 이후 CT에서는 scan 범위에 충분히 포함되지 않았음 따라서 Extrinsic mass에 의한 compression의 가능성을 완전히 배제할 수는 없음 또한 Lower thoracic esophagus에 multifocal, nonperistaltic tertiary contraction이 발생함 GE junction은 normal shape을 보이며, contrast passage disturbance 소견 없음 Pharyngography에서 특이 소견 없음 [CONCLUSION] Nonspecific motility disorder. Marked luminal narrowing in mid-thoracic esophagus (Idx3, Arrows) with resultant contrast passage disturbance & reflux to upper esophagus. → DDx. Extrinsic compression by paraesophageal mass or esophageal subepithelial tumors, etc. Multifocal nonperistaltic tertiary contractions involving lower thoracic esophagus. Normal shape of GE junction without evidence of contrast passage disturbance. Normal pharyngography.
